An unannounced monitoring inspection was conducted today. There were 55 children present with 12 staff.
The children were playing outdoors, resting and working independently .
Five child records and five staff records were reviewed.

Based on the review of five staff records, the facility failed to ensure that all required background checks are completed for all employees.
Evidence:
1. Staff #4 indicated on the sworn statement that they had lived in another state in the past five years. There is no out-of-state central registry clearance on file for staff #4.

Based on a review of five staff files, the facility failed to ensure that all staff submit a negative TB screening at the time of employment and prior to coming into contact with children.
Evidence:
1. There are no TB screening results on file for staff #1 & 5. Hire dates: 8-16-22 & 7-11-22.
