An announced monitoring inspection was initiated on 9/23/25 and completed on 9/30/25 in response to a self-report received by the licensing office on 9/23/25 relating to staffing and supervision. This inspection included document review, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Forbidden actions such as physical punishment, striking a child, roughly handling or shaking a child, restricting movement through binding or tying, forcing a child to assume an uncomfortable position, or exercise as punishment.
On 09/23/25 the center self reported that staff #1 had roughly handled child #1 (preschool aged), during a child on child incident, by grabbing child #1 by the wrist and the back of the neck.

Plan of Correction: Center administration immediately investigated the incident, notified required agencies, and followed internal incident procedures and policies. The staff member was immediately suspended and then employment was terminated.
