Inspector Notes

A renewal inspection was conducted on 04/03/2023. There were 10 children present with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed.

Information gathered during the inspection determined a non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Based upon review of children's records and staff interview, the facility has not ensured that each child has a physical examination by or under the direction of a physician within 30 days of attending.
Evidence:
1. The record provided for child 5, who began attending on 2/27/2023, did not include the results of a physical examination.
2. Staff 1 verified that the record did not yet include the results of a physical examination,
Plan of Correction: The facility responded with the following:
We have been working with the placement agency for child 5 to obtain a physical examination. Results of a physical examination was obtained on 4/3/2023.
