Inspector Notes

A monitoring inspection was conducted on 05/19/2022, from 10:30am to 11:20am. There were 14 children present and three staff. The Inspectors reviewed five children?s records and three staff records. The Inspectors observed staff and children?s interactions throughout the inspection. Children were observed during circle time, free play, and group activity. First aid kits and emergency preparedness documents were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the site manager at the exit interview.

Standard 8VAC20-780-160-C
Based on record review and interview, it was determined that the center did not ensure that at least every two years from the date of the initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening.
Evidence:
1) Staff #1 has a tuberculosis screening with a last known date of 12/5/2019, and Staff #2 has the last known date of 11/4/2019, which is outside of the required timeframe of two years and that neither Staff #1 nor Staff #2 had documents to indicate that they had completed the required screening.
2) Administration acknowledged that a new TB screening was required two years from the date of the last screening.
Plan of Correction: The center responded with the following: Administration will inform Staff #1 and Staff #2 of the required tuberculosis screening and to submit documentation to the center.
Standard 8VAC20-780-60-A
Based on record review and interview, the center did not ensure that the work phone number and place of employment of each parent who has custody of the child is listed in the record.
Evidence:
1. The record for Child #1 did not have the work name and work phone number for their parent.
2. Staff #3 confirmed that the parent for Child #1 was employed and her work information was not listed in the child?s record.
Plan of Correction: The center responded with the following: The center will obtain the parent?s work name and phone number for Child #1 at pick up.
