Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on September 23, 2025. The on-site inspection began at 9:35am and ended at 10:45am. The inspector reviewed compliance in the areas listed above. There were 10 children present and 3 staff. The inspector reviewed 5 children?s records. This inspection included document review tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Where playground equipment is provided,
resilient surfacing shall comply with minimum
safety standards and shall be under equipment
with moving parts or climbing apparatus to
create a fall zone free of hazardous obstacles.
The mulch is measuring about an inch in depth
on the outdoor playground and under the slides
the children were landing in patches of dirt and
soil.
Plan of Correction: The center responded with the following: The center has emailed and informed the housing complex of the need for additional mulch for the outdoor playground. The center wanted it to note that there are neighborhood children that are using the playground as it is part of the housing complex. Staff will email and inform the housing development again as well as the supervisor for MHK regarding the need for additional resilient surfacing. MHK has also priced rubber mulch for the playground should the housing complex not replenish the mulch.
