Inspector Notes
An unannounced investigation was initiated on 4/3/25 and completed on 4/21/25 in response to a complaint received by the licensing office on 3/31/25 relating to allegations of supervision, care, protection, and guidance, and behavioral guidance. The inspector reviewed three children?s records, and one staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation supports the allegation of supervision and care, protection, and guidance; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/5/25. A POC submitted after this date will not appear on the public website.
Violations
2Evidence: On 3/28/2025 at approximately 10:15am, Staff #1 was supervising 13 children, ranging in age from three-years-old to school age, in the multi-purpose room. Child #1, three-years-old, began poking other children in the group, Child #2, three-years-old, who was poked chased after Child #1 and pulled the child?s hair causing two braids to be pulled out of their scalp.
Updated supervision procedures were reviewed during a staff meeting given by owner on 4/24/2025 and now require consistent positioning and scanning during transitions.
Evidence: On 3/28/2025, there were 13 children with one staff in the multi-purpose room from approximately 10:15am to 10:20am. The youngest child in the group was three-years-old.
