During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced investigation was initiated on 4/3/25 and completed on 4/25/25 in response to a complaint received by the licensing office on 4/2/25 relating to allegations of sight and sound supervision, clean, safe, and operable, and injury reporting. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation supports the allegation of sight and sound supervision; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Children under 10 years of age, shall be within actual sight and sound supervision of staff at all times.

Evidence: When the three-year class utilize the restroom in the multi-purpose room after outside time, staff remain at the restroom and send children to hang up their coats in the three-year class. The three-year classroom is located outside of the multi-purpose room across from the Directors desk in the lobby. Interviews revealed that the Director is not always at their desk therefore, children from the three-year class were not within sight and sound supervision at all times.
Plan of Correction: A revised staff schedule ensures compliance with ratio requirements at all times, with a dedicated floater available to support as needed. Will hire a floater to cover ratios by May 2, 2025. Director will fill in until position is filled if needed.

Ratio counts are checked every hour via brightwheel app, overseen by administration.

Staff have been reminded to alert admin immediately if a room approaches or exceeds ratio limits.
