Inspector Notes
An unannounced monitoring inspection was conducted 3/24/2023 at 12:05pm. There were 70 (infants to pre-k) directly supervised by 11 staff. The physical plant, 3 staff records, 5 children?s records, 2 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed preparing for and taking naps. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Violations
6Evidence: reviewed 3 staff records and found that Staff 1 and 3 did not have documentation of orientation training.
Evidence: observed in the infant kitchen, the upper kitchen cabinet that was only attached by a single hinge, and resting on the cabinet door next too it.
Evidence: observed that there was Lysol spray accessible to children in an unlocked cabinet in the First Steps classroom.
Evidence: the facility did not have any documentation of when the most recent injury prevention plan update took place.
Evidence: observed in the Toddler classroom, children's nap cots were within 12 inches of each other.
Evidence: reviewed 3 staff records and found that Staff 1 and 2 did not have documentation of 2 reference checks prior to employment.
