Inspector Notes

An unannounced monitoring inspection was conducted 3/24/2023 at 12:05pm. There were 70 (infants to pre-k) directly supervised by 11 staff. The physical plant, 3 staff records, 5 children?s records, 2 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed preparing for and taking naps. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Based on review, the facility did not ensure that there was documentation of orientation training in each staff's record.

Based on observation, the facility did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence: observed in the infant kitchen, the upper kitchen cabinet that was only attached by a single hinge, and resting on the cabinet door next too it.

Based on observation, the facility did not ensure that hazardous substances are kept in a locked place using a safe locking method that prevents access by children.
Evidence: observed that there was Lysol spray accessible to children in an unlocked cabinet in the First Steps classroom.
