Inspector Notes
An unannounced monitoring inspection was conducted on 9/26/2023 at 1:40pm. There were 65 (Infants to 5yrs) directly supervised by 10 staff. The physical plant, 3 staff records, 5 children?s records, reviewed 4 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed napping and interacting with staff. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Violations
4Evidence: reviewed 5 children's records and found that Child 3 did not have documentation of a physical examination.
Evidence:
1) observed sensory bottles, in the Get Set classroom, with "glittery" liquid inside, with lids that are not sealed, making the contents accessible to children.
2) observed in the Get Set room, bleach water in an unlocked cabinet.
3) observed in the multipurpose room, a spray bottle with soap water on top of the children's cubbies.
4) observed in the Preschool 2 room, soap water spray bottle accessible to children, on the floor, in an unlocked cabinet.
Evidence: observed a sensory bottle, in the Get Set room, that was not sealed shut, with small plastic shapes and jewels accessible to children under 3 years of age.
Evidence: reviewed 5 children's records and found the following:
1) Child 3 and 4 did not have a second emergency contact.
2) Child 5 did not have addresses for either emergency contact.
3) Child 4 and 5 did not have documentation of work information for either parents.
4) Child 4 and 5 did not have documentation of parent's annual review of emergency information.
