Inspector Notes

An unannounced monitoring inspection was initiated on 3/21/25 and completed on 4/9/25 with the local department of social services in response to a self-report received by the licensing office on 3/18/25 relating to Supervision. The inspector reviewed 1 child's record and 2 staff records on-site on 3/21/25. This inspection included document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, a violation is documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/16/25 A POC submitted after this date will not appear on the public website.

Children under 10 years of age always shall be within actual sight and sound supervision of staff.

On 3/18/25 at approximately 10:35am, staff A left child A, 3 years of age, alone in the classroom bathroom while the staff took a group of preschoolers outside to play. Child A was left alone in the room for approximately 25 minutes.
Plan of Correction: Teacher was terminated immediately. Child found safe, no signs of distress and remains enrolled. Retrained all staff.
