Inspector Notes
A monitoring inspection was completed from 1:50 p.m. until 3:00 by two inspectors. There were 12 children present, ranging in ages from infant 5 yrs. old for a total of 30 pts. with two providers and one household member present. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 3 child records and four 4 provider and/or household member records were reviewed. Children were observed resting and being fed a bottle.
The inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on interview, the substitute provider failed to maintain a record of arrivals and departures on each day that the substitute provider works.
Plan of Correction: The substitute provider will start to document hours.

Based on observation, the provider failed to keep poisonous substance supplies inaccessible to children.
