Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.

A monitoring inspection was initiated on 3/15/2022 and concluded on 3/16/2022. The provider was contacted by email and a virtual inspection was conducted. The inspector reviewed compliance in the areas of administration, staffing and supervision, programming, medication (provider not trained to administer medication and no medication kept on-site) special care and emergencies and nutrition. A total of 4 child records, 2 caregiver records and 1 household member record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview, the provider has not obtained a subsequent tuberculosis (TB) screening for each caregiver and adult household member every two years.
Evidence:
1. The providers most recent TB screening is dated 1/26/2020.
2. Household member #1's most recent TB screening is dated 1/23/2020.
Plan of Correction: We have appointments scheduled to get updated screenings.
