Violations

5
Standard 8VAC20-800-320-B
Based on observation, protective covers were not installed on all unused electrical outlets accessible to children under five years of age.
Evidence: There were 2 outlets observed near the couch, 2 outlets observed next to the lamp shelf, 2 outlets observed in the nap room and 1 outlet observed in the hallway, all without protective covers installed.
Plan of Correction: I can't put the covers in. I guess I will have to get new plugs.
Standard 8VAC20-800-480-D
Based on observation equipment was not assembled and maintained in accordance with the manufacturer's instructions.
Evidence: A crib was observed in the nap room with the base and mattress sitting directly onto the floor. The crib was not assembled per the manufacturers safety instructions. A blanket and Minnie mouse stuffed animal were observed in the crib.
Plan of Correction: I don't know why it's like that. I'll fix it today.
Standard 8VAC20-800-570-A
Based on observation and interview, the provider did not ensure that a caregiver did not exceed 16 points.
Evidence:
1. On 7/14/2023, caregiver #2 was observed working alone with 9 children (25 points); Child #1, 13 months. Child #2, 15 months. Child #3, 7 months. Child #4, 6 months. Child #5, 4 years. Child #6, 2 years, Child #7, 5 years, Child #8, 2 years. Child #9, 2 years.
2. Caregiver #2 stated that the provider was not home. The provider confirmed that they were traveling outside side of the state on the date of the unannounced inspection.
Plan of Correction: I never leave but I had a family emergency. I'm trying to hire additional help so that I can leave the house without leaving [caregiver #2] over on points. I won't do it again.
Standard 8VAC20-800-920
Based on observation, a weekly food menu was not posted.
Evidence: Caregiver #2 was observed feeding the children an afternoon snack (bananas and crackers). There was no food menu posted.
Plan of Correction: Corrected.
Standard 8VAC20-800-960-E
Based on observation, bottles were not labeled with each child's full name and date when more than one infant was in care.
Evidence: Three bottles were observed sitting on high chairs. One bottle was not labeled with a name or date. Two bottles were not labeled with the date.
Plan of Correction: I'll have the parents start putting the name and date.
