During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 7/10/2025. The on-site inspection began at 11:15 AM and ended at 12:10 PM. The inspector reviewed compliance in the areas listed above. There were 7 children present and 2 caregivers; the point total was 16. The inspector reviewed 7 children?s records and 3 caregiver/household member records. This inspection included document review, a tour of the family day home, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 7/17/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and adult household member a subsequent tuberculosis (TB) screening every two years from the date of the first screening.

1. Caregiver #1's most recent TB screening is dated 6/20/2023.
2. Caregiver #2's most recent TB screening is dated 6/23/2023.
3. Household member #1's most recent TB screening is dated 6/20/2023.
4. The provider confirmed that updated TB screenings had not been completed.
Plan of Correction: Will get it done for all providers.
Standard 8VAC20-800-320-B
Child-resistant protective covers shall be installed on all unused electrical outlets accessible to children under five years of age.

Two unused outlets were observed in the children's play area without protective covers. Seven out of seven children observed in care are under the age of five.

REPEAT VIOLATION
Children's records shall contain all required information.

Child #1's record does not contain the addresses of two emergency contacts or the families medical insurance policy number.

Topical skin products such as diaper ointments shall be labeled with each child's name.

An unlabeled diaper cream was observed at the diaper changing table. The provider stated that the diaper cream belonged to child #1.

A working portable battery-operated weather band radio, and extra batteries shall be kept in a designated area and be available to caregivers at all times.

The providers battery-operated weather band radio did not work when turned on.
Plan of Correction: Ordered a new one.
