During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was completed on 09/26/2025, as a part of the licensure period. The on-site inspection began at 11:35 am and ended at 12:45 pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and 4 caregivers; the point total was 39. The inspector reviewed 4 children?s records and 6 caregiver/household members records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/03/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-60-B
A child's record shall contain proof of the child's age and identity, as well as written authorization for emergency medical care.

Child #1 did not have complete documentation of their age and identity in the record, and child #3 did not have authorization for emergency care.
Plan of Correction: Got the parent to fill it, and bring an original of the birth certificate for me to view.
Standard 8VAC20-800-630-A
Caregivers shall use positive methods of discipline. Discipline shall be constructive in nature, including techniques such as modeling and redirecting children to acceptable behavior.

During the inspection, a caregiver was seen forcefully pushing a child's head to their cot.
Plan of Correction: Spoken to the aide about naptime routine and helper her to understand what is proper handling of a child.
