Inspector Notes
The licensing inspector conducted an inspection 11/09/2023 regarding a complaint of a child being left on the playground on 09/15/2023 unsupervised. Observations were made, interviews were conducted, and documentation was reviewed. The preponderance of the evidence gathered during the inspection supported a violation of the Standards for Licensed Child Day Centers.
Please sign, complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Evidence:
According to interviews and center documentation, child, C1, age 3, was reported to have been left alone on the playground on 09/14/2023 for less than 5 minutes. The classroom teacher, S1 did not know the child was alone on the playground until she did a head count in the classroom and realized she was missing one child. Another staff then radioed that the child was outside on the playground. The child was retrieved from outside and returned to the classroom.
