Inspector Notes
An unannounced monitoring inspection was conducted on February 1, 2024 between the hours of 9:06am and 12:02pm. There was a total of 80 children with 18 staff members supervising. Children were observed engaged in group activities, story time, various age appropriate games and materials, outdoor play time, diapering, handwashing, sleeping and eating lunch. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, children and staff records, special care and emergencies and nutrition. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. 4Evidence: The record for Staff #1 did not have documentation of an updated criminal history check (expired on 12/27/23).
Evidence:
1. Staff #5 in the nursey classroom did not wash their hands before a diaper change.
2. Staff #5 in the nursey classroom did not wash their hands before helping a child with feeding their bottle.
3. Staff #6 in the nursey classroom did not wash their hands before a diaper change.
Evidence:
1. Staff in the nursey classroom were not using disposable towels to clean the diapering area.
2. The leakproof disposal container in the nursey classroom was broken. The container would not open and close using the foot-operated pedal.
3. The diapering surface in the nursey and the Active Asteroids classroom (1-2 year old's) was not cleaned with soap after each diaper change.
Evidence:
1. In the nursey classroom, child #6 had a tube of A&D ointment that expired in 11/2023.
2. In the nursey classroom, child #7 a tube of Aquaphor expired in 11/2023.
