Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on October 21, 2024. The on-site inspection began at 10:30am and ended at 12:00pm. The inspector reviewed compliance in the areas listed above. There were 73 children present and 20 staff. The inspector reviewed 6 children?s records and 6 staff records on-site. This inspection included document
review, tour of the facility and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain all the required information.
The record for child #1, enrolled for 7 weeks, was missing the documentation of reviewing the proof of identity.
Plan of Correction: I will make sure to view and document the proof of identity by the first day of attendance.
Standard 8VAC20-780-90-A
A written agreement between the parent and the center shall be in each child's record by the first day of the child's attendance. The agreement shall be signed by the parent.
The record for child #1, enrolled for seven weeks, does not have documentation of a signed agreement by the parent.
Plan of Correction: I will have the parent sign it today and make sure all agreements are signed by the first day of attendance.
