Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on 06/03/2021 and concluded on 06/10/2021. The director was contacted by telephone to initiate the inspection. There were 65 children present and 12 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.
The information gathered during the inspection determined non-compliance with subsidy regulations, and violations were documented on the violation notice issued to the facility.
Violations
2Evidence: 1) A physician letter dated 12/08/20 in the record for Child #6 indicated the child has a diagnosed food allergy, but the record did not contain a written allergy care plan. 2) During interview, a member of management confirmed the center does not have a written allergy care plan for Child #6. A child's record shall include a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence: 1) The TB screening in the record for Staff #1, hired on 01/25/2021, was dated 02/06/21. 2) The TB screening in the record for Staff #4, hired on 05/12/21, was dated 03/20/21. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
