Inspector Notes
The licensing inspectors conducted an unannounced complaint inspection in response to a complaint received on 01/10/2023 regarding allegations concerning supervision of children. The inspection was completed on 01/24/2023. The inspectors were on site from approximately 11:26 AM to 1:51 PM. Interviews with staff were completed and documentation reviewed.
The preponderance of the evidence gathered during the investigation did not support the allegation. The complaint is determined to be not valid; however, violations of standards were determined. See the violation notice for details.
Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

The incident/accident report for C1 did not document how the parent was notified of the injury. The element on the report was blank.
Evidence:
The record for C1, enrolled 11/16/2022 did not contain a second emergency contact for the child if the child?s parent could not be located.
Evidence:
The center did not notify the Virginia Department of Education of a child injury that occurred on 11/22/2022. The Director provided a copy of notification made to the Virginia Department of Social Services. The center has been licensed by the Virginia Department of Education since 07/01/2021.
