8VAC20-780 Staffing and Supervision

Inspector Notes

An unannounced inspection was initiated on 06/27/2024 and completed on 07/08/2024 with local agencies, in response to a complaint received by the licensing office relating to allegations of staffing and supervision. The inspector reviewed 2 children?s records and 1 staff records on-site.
This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/11/2024. A POC submitted after this date will not appear on the public website.

When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 6/19/24 S1 was diapering a child in the classroom bathroom, and she heard C1 crying in the classroom. She reported when she finished diapering the child and entered the classroom, she observed C1 crying and that C1?s face and creeks were red. S2 documented in the incident report hearing a child crying and entered the classroom to see what was going on. S2 noticed another child laying on the side of C1. S2 called the child?s name, and the child jumped off C1. S1 reported she picked up C1 and took her out of the classroom and noticed scratches on her face which she washed and cleaned.

Children under 10 years of age shall always be within actual sight and sound supervision of staff. On 6/19/2024, S1 was in the 2-year-old classroom bathroom changing a child, and heard C1 crying in the classroom. S1 reported she was the only teacher in the classroom with the 8 children. S1 reported that while she had the bathroom door open, she could not see the children in the classroom.
