Inspector Notes

An unannounced inspection was completed on 11/12/2024 in response to a complaint received by the licensing office on 11/06/2024 relating to allegations of physical plant and supervision of children. This inspection included observations and staff interviews.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 02/27/2025. A POC submitted after this date will not appear on the public website.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. During observations in the 2-year-old room, infant B, toddler room, and preschool II-B classroom toys were strewn across the classroom floors creating trip and falling hazards. During observations in the toddler room, preschool II-A, pre-school II-B the brooms and dust pans used to clean the classrooms were observed to be caked with food and dirt debris and accessible to children. The kitchen cart used to transport food to the classrooms the bottom shelf was observed to be caked with food and liquid debris. The sofa cushions in the pre-k-A room were ripped.
