The center shall ensure that areas and equipment of the center, inside and outside,
are maintained in a clean, safe and operable condition. A rip was observed in the
resilient surfacing on the playground creating a trip hazard. Floor molding of Pre-school 1-A was peeling off the wall in the bathroom. Peeling wall covering and frayed carpet in the play pit were observed in City Place play area.

required information. Reports dated 08/04/2025, 09/16/2025, 09/24/2025, and 10/1, 6, 7/2025 did not document how parents were notified of the injuries. Reports dated 08/04/2025, 08/11/2025, 08/13/2025, 08/17/2025, 08/19/2025, 09/16/2025, 09/24/2025, 10/01/2025, 10/06/2025, and 10/07/2025 did not document the time of parental notifications. Report dated 08/17/2025 did not document any future actions to prevent recurrence of the injury.

When food is brought from home the food container must be clearly dated. Two water bottles observed in the 2-A classroom were not dated.

I sent notification to the parents to let them know that it is a violation to not list the full name and date of the water bottles. I also let them know that if this information was not listed on the water bottle their child would not have access to the water bottle throughout the day.
