Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A subsidy health and safety inspection was initiated on and concluded on 05/18/21. The provider was contacted by telephone to initiate the inspection. There was 1 child present and 1 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance with subsidy regulations, and a violation was documented on the violation notice issued to the facility.

Evidence:
The update training was not completed on 2020. Certificates are dated 2019, and 2021.
