Inspector Notes

An in person renewal study was conducted today. There were 2 children present (3 points) with 1 caregiver. 2 child, 2 dog's, 2 caregiver and one household member records were reviewed. See Violation Notice for non-compliances. The Virginia Safety Inspection for the van expired yesterday. The first aid supplies, menu, emergency supplies, emergency evacuation drills, bathroom supplies and the fridge/freezer temperature are all in compliance. LI was present today from 1:45 pm-3:00pm. Based on observation the provider failed to ensure that the vehicle used for transportation:
1. Meets the safety standards set by the Virginia Department of Motor Vehicles.
Evidence:
The inspection sticker on the vehicle is dated 5/22 (expired).
Plan of Correction: I will get the van inspected by this weekend.

Based on a review of records, on staff record does not contain documentation of all required components.
Evidence:
1 staff record reviewed did not contain:

Plan of Correction: I will have the staff person provide a new reference.
I will ask her for the copy of her High School diploma, and I will help her complete the form.

Based on a review of records, the dogs are not vaccinated for rabies.
Evidence:
2 of 2 dog rabies vaccinations reviewed were expired.
*Dog 1 - vaccination expired 12/19/21.
*Dog 2 - vaccination expired 12/27/21.
Plan of Correction: I will take the dogs to get their vaccinations.
