Inspector Notes

An unannounced inspection was completed on 01/10/2025 in response to a self-reported incident relating to allegations of Supervision. The inspector reviewed children and staff records. This inspection included: document review, tour of the facility, interviews, observations and measurements. The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program. Though not a part of the original self-report violations additional violations were found during the monitoring inspection and are documented on the violation notice issued to the program.

Standard 8VAC20-780-340-F
Children under 10 years of age always shall be within actual sight and sound supervision of staff.

On 12/04/2024 when coming in the building after outside play around 10:50am, staff #1 left a 2year-old child #1 outside on the playground with no sight and/or sound supervision for about 8 minutes.
Plan of Correction: The incident was self-reported and the staff's employment was terminated. All staff have been retrained on playground procedures and sight and sound supervision. An additional layer of checks and balances was added to the playground procedures to include a face-to-name sheet.
Standard 8VAC20-780-370-3
Infants are required to be provided with daily outdoor time if weather and air quality allow based upon the Air Quality Color Code Chart as provided by the Department of Environmental Quality.

The center has not been providing infants with daily outside time. The director confirmed since her employment over 4 months ago, they have not been outside.
Plan of Correction: The director will work with infant staff to come up with a plan and schedule to ensure that infants are provided outside time daily.
