Inspector Notes
An unannounced, on-site monitoring inspection was completed on 04/01/2025. The on-site inspection began at 10:16 a.m. and ended at 4:30 p.m. The inspector reviewed compliance in the areas listed above. There were 93 children present and 22 staff. The inspector reviewed 10 children?s records and 11 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 06/09/2025. A POC submitted after this date will not appear on the public website.
Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of
employment, before coming into contact with children, and shall have been completed within
the last 30 days of the date of employment. S3 TB was dated a day after the date of employment.
Plan of Correction: Teachers will be required to send or bring proof of TB test prior to first date of employment. If a TB screening does not fit the requirements, their start date will be pushed back until a sufficient screening is obtained.
Standard 8VAC20-780-240-A
Staff shall complete the Virginia Department of Education-sponsored orientation coursed within 90 days calendar days of employment. S2 and S7 completed the course 4 months after the date of employment.
Plan of Correction: S2 needed extra time due to English being a second language. The leadership team will continue to make training outside of the classroom a priority including the state-sponsored course.
Standard 8VAC20-780-270-C
The inside area occupied by children shall be maintained no lower than 68 degrees Fahrenheit. The thermostat in the Toddler I room showed the temperature in the classroom at 67 degrees.
Plan of Correction: The HVAC unit has since been repaired to ensure appropriate temperatures in the classroom.
Standard 8VAC20-780-340-F
Children under 10 years of age shall always be within actual sight and sound supervision of staff. During the inspection, the licensing inspector observed a toddler playing behind a utility shed on the playground out of the actual sight and sound of staff.
Plan of Correction: Teachers were immediately re-trained regarding playground safety and positioning on the playground. The leadership team will continue to do observations on the playground and coach in the moment.
Standard 8VAC20-780-500-A
Children?s hands shall be washed with soap and running water after toileting, or any contact with blood, feces or urine. Licensing inspector observed a staff member in the Early Preschool 1 classroom changed three children?s diapers in the toddler room and did not wash the children?s hands afterwards with soap and water.
Staff shall wash their hands with soap and water before and after a diaper change. Licensing inspector observed a staff member in the Early Preschool 1 classroom change multiple children?s diapers and not wash her hands before and after each diaper change.
Plan of Correction: The teachers in question were immediately re-trained regarding the handwashing expectations. At the next staff meeting, the entire staff was re-trained on cleaning and hand-washing expectations.
Standard 8VAC20-780-500-B
Staff shall dispose of diapers without the staff member?s hand touching the exterior surface of the storage system during disposal. Licensing inspector observed a staff member in the Early Preschool 1 class lift the top of the diaper disposal can with her gloved hands used to change diapers to dispose of dirty diapers.
The diapering surface shall be cleaned with soap and sanitized after each use. Licensing inspector observed a staff member in the Early Preschool 1 class failing between multiple children being diapered to clean with soap the diapering surface before sanitizing the diapering surface.
Plan of Correction: A new, touchless trashcan has already been put into place. The teachers in question were immediately re-trained regarding the cleaning expectations. At the next staff meeting, the entire staff was re-trained on cleaning and hand-washing expectations.
Standard 8VAC20-780-560-G
When food is brought from home the food container must be clearly dated. Multiple
thermos bottles brought from home were not dated in the Toddler I and Toddler II rooms.
Plan of Correction: The thermos? in question were water bottles and the team had not been ensuring they were dated. Moving forward, we understand the expectation and will ensure they are all dated.
Standard 8VAC20-780-70
Staff record shall contain documentation to demonstrate that the individual possesses the
education, certification, and experience required by the job position. The staff records for S6 and S8 identified as a lead did not document the qualifications to be a program lead.
Plan of Correction: Staff S6 no longer works at the center. Staff S8 has the necessary documentation to qualify her as a lead including a degree and sufficient trainings upon hire. The leadership team will continue to ensure that all program leads have the necessary training and education necessary.
