Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of
employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. The TB screening for S4 was dated more than 30 days before employment.
Plan of Correction: Teachers will be required to send or bring proof of TB test prior to first date of employment. If a TB screen does not fit the requirement, their start date will be pushed back until a sufficient screening is obtained.
Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening. S7?s
updated screening was completed more than 7 months late.
Plan of Correction: Teachers will be required to provide proof of TB test screening within 2 years of the previous screening. If a TB screening is not done with the 2 year time frame, staff will no be permitted to work and placed on leave until screening is obtained.
Standard 8VAC20-780-240-B
Prior to working alone with children and within seven days of the first day of employment, staff shall complete orientation training required by 8VAC20-780-240-C. The orientation forms for S3 and S4 did not verify the specific facility topics.
Plan of Correction: Teachers will initial, sign and date their completed orientation document appropriately.

When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up within 14 days shall be disposed of by the center. The authorization for an over-the-counter medication for C11 expired 09/10/2025 and the medication was observed during the licensing inspection.
Plan of Correction: When an authorization for medication expires, the parent shall be notified via email that the medication needs to be picked up within 14days or the parent must renew the authorization.
