Inspector Notes
A monitoring inspection was conducted on October 13, 2022 from 2:20 pm until 1:35 pm. The inspector reviewed compliance in the areas of the license, playground, injury prevention and diapering procedures.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on observation, program failed to ensure that the Notice of the Superintendent's intent to take any of the actions enumerated in subdivisions B 1 through 6 is posted in a prominent place at each public entrance of the licensed premises to advise consumers of serious or persistent violations.
Evidence:
1. Upon arrival at the facility on 10/13/2022, the Notice of Intent (NOI) was not posted at the entrance.
2. The Director confirmed that it was not posted, and that she did not have a copy of the NOI to post.
3. The licensee confirmed electronic receipt of the NOI on 8/29/2022.
