Inspector Notes
The licensing inspector initiated an unannounced inspection on 08/22/23 and concluded on 08/22/23 in response to a complaint received by the department on 08/18/23 for allegations regarding supervision and physical plant. Interviews and observations were conducted relating to the allegations. The information gathered during the investigation support the allegations in part, so the complaint is determined to be "valid." Areas of non-compliance are identified on this Violation Notice.

Based on observation and interviews, the center failed to ensure the playground was designated to protect children from hazards.
Evidence:
1. Staff 1 stated during COVID a temporary fence was put up for smaller group sizes. Staff 1 also stated the fence was intended to contain children and keep the community out as there is a public walk path and parking near by.
2. Per the mother of child 1, she was advised on 8/17/23 her child and two other children lifted the temporary fencing and attempted to leave the playground.
3. Staff 1 and staff 2 stated the three children were seen lifting the plastic fence. Two were caught before getting through and one was caught upon standing.
4. The plastic temporary fencing is loose enough to be lifted from the bottom so that children and staff can fit under. At the front of the building the fencing has a hole at the bottom that a child could fit through.
