Inspector Notes

A monitoring inspection was conducted on 06/17/2024 from 9:22 AM to 1:55 PM. There were 98 children present, ranging in ages from three months to twelve years, with 18 staff. The inspector reviewed compliance in the areas of administration, staff qualifications and training, physical plant, staffing and supervision, programming, medication, and special care and emergencies. A total of nine children?s records, two children?s medications, and the nine staff?s records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review and interview, the center failed to obtain documentation of physical examinations and immunizations that were signed by a physician, his designee, or an official of a health department.

Evidence:

The record for child 9 did not contain an immunizations and physical examination that was signed by a physician, his designee, or an official of a health department.
Plan of Correction: The assistant director will create all children's files. The intake checklist will be verified by another leadership team member, ensuring compliance prior to enrollment.

Based on record review and interview, the center failed to implement a monthly practice evacuation drill.

Evidence:

The documentation for the monthly practice evacuation drills did not include documentation for the month of April 2024. The director stated they did not completed an evacuation drill in April 2024.
Plan of Correction: The center held two drills in May. The center director met with the facility emergency manager responsible for drills on campus. A meeting has been scheduled for the last Friday of the month to either confirm the monthly drill took place or to conduct it.
