Inspector Notes

An unannounced monitoring inspection was conducted today between the hours of 1:10pm and 2:20pm. There were 4 children directly supervised by the provider and one additional caregiver. The home, 2 caregiver records, 4 children?s records, 2 household member?s records, evacuation drills, and emergency supplies were inspected. There were no medications on site today. Children were observed during naptime. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Based on record review, the family day home did not obtain the results of a fingerprint based criminal history record check every 5 years for 3 of 4 caregiver/household members.

Evidence:
1. New fingerprint check results were obtained for Caregiver #1 on 6/9/2023, more than 5 years after the previous fingerprint check results dated 2/14/2018.
2. New fingerprint check results were obtained for Household member #1 on 6/9/2023, more than 5 years after the previous fingerprint check results dated 2/14/2018.
3. New fingerprint check results were obtained for Household member #2 on 6/9/2023, more than 5 years after the previous fingerprint check results dated 2/14/2018.
Plan of Correction: The family day home will obtain fingerprint check results for all caregivers and household members prior to 5 years from the previous fingerprint check results.

Based on record review, the provider did not obtain documentation of a physical examination within 30 days after the first day of attendance for one child.

Evidence:
1. The record for Child #2 (start date 12/5/2022) did not contain documentation of a physical examination.
Plan of Correction: The family day home will request documentation of a physical from the parents.

contact in case of an emergency if the parent cannot be reached
2. The record for Child #4 did not contain the address of the place of employment for a second custodial parent.

Based on record review, the provider did not review the emergency contact information required in the child's record with the parent annually for 2 of 4 children reviewed.

Evidence:
1. The most recent review of emergency contact information in the record for Child #1 (start date 8/16/2021) was dated 3/14/22.
2. The most recent review of emergency contact information in the record for Child #4 (start date 2/1/2021) was dated 3/15/2022.
