During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 2/27/2025, as a part of the licensure period. The on-site inspection began at 11:10am and ended at 12:10pm. The inspector reviewed compliance in the areas listed above. There were 4 children present and 2 caregivers; the point total was 8. The inspector reviewed 4 children?s records and 4 caregiver/household member's records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. There were no physical exam records available for Child #1, who had been in care for 6 months.
Plan of Correction: Provider will request documentation from the parents.
Standard 8VAC20-800-110-B
Reports of immunizations shall include the dates immunizations were received and shall be signed by a licensed physician, the physician's designee, or an official of a local health department. The immunizations in the record for Child #1, who has been attending the family day home for 6 months, were not signed by a licensed physician, the physician's designee, or an official of a local health department.
Plan of Correction: Provider will request from the parents immunizations signed by a physician or designee.
Standard 8VAC20-800-210-A
Repeat Violation
Caregivers shall obtain a minimum of 16 clock hours of training annually. Documentation of training hours completed in 2024 were not available for review during the inspection for Caregiver #1 and Caregiver #2.
Plan of Correction: Provider will submit documentation of completion.
Standard 8VAC20-800-60-B
Children's records are required to contain written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately. The record for Child #3, who has been in care for over a year, did not contain written authorization for emergency medical care.
Plan of Correction: The provider will request the parent's signature.
Standard 8VAC20-800-80-A
Within seven business days of the child's first day of attendance, the provider shall obtain from the parent verification of the identity and age of the child. This information was not available for Child #3, who has been in care for over a year.
Plan of Correction: The provider will request the information and document in the record.
