During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 7/7/2025, as a part of the licensure period. The on-site inspection began at 11:30am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers; the point total was 13. The inspector reviewed 5 children?s records and 4 caregiver/household member's records on-site.

This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
A tuberculosis (TB) screening is required every two years from the last screening. An updated TB screening for Caregiver #1, Household Member #1 and Household member #2 were not completed and were overdue by two months.
Plan of Correction: Caregiver and household members will get new tb screenings.
Standard 8VAC20-800-700-C
When an authorization for medication expires, the parents shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications not picked up by the parent within 14 days shall be taken to a pharmacy for proper disposal. Medication for Child #4 was present in the family day home. The authorization for the medication expired eight months ago.
Plan of Correction: Provider will give the medication to the parent.
Standard 8VAC20-800-710
The family day home may administer prescription medication provided the medication is maintained in the original, labeled container; and the caregiver administers drugs only to the child identified on the prescription label. Medication stored in the family day home for Child #1 was not in the original labeled container and was not labeled with the child's name.
Plan of Correction: Provider will request the medication be provided with the original packaging and label from the pharmacy. Provider will ensure all medication is labeled with the child's name.
Standard 8VAC20-800-90-C
Documentation of additional immunizations is required once every six months for children under the age of two years. The last documented immunization for Child #3, 13 months old, was over 11 months ago.
Plan of Correction: Provider will request updated immunizations.
