Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A complaint inspection was initiated on 03/30/2021 and concluded on 05/19/2021. A complaint incident was received by the department regarding allegations in the areas of behavior management. The director or in-charge person was contacted by telephone to conduct the investigation in conjunction with a local agency. The inspector emailed the director a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported the allegation of non-compliance with standards or law and violations were issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Violations
3Evidence:
1. Child #1 was pulled away from the table by Staff A. Child #1 fell out of the chair and landed on face which resulted in busting lower lip.
2. Classroom footage was reviewed and incident was watched at time stamp 11:58:48 AM
3. Staff A verified she pulled the chair away from the table to stop Child #1 from playing with food on the table.
4. Staff A was observed on classroom footage taking a book out of a child's hand and throwing it on the carpet. Time stamp: 11:53:32 AM
5. Staff A was observed on classroom footage grabbing a child by the arm to bring them out of the bathroom. Time stamp: 11:58:14 AM.
Evidence:
1. A staff member was observed in video footage at time stamp 11:55 AM not washing hands with soap and running water before and after changing a child's diaper.
2. Administrator verified that Staff A did not wash hands before and after diaper changing.
Evidence:
1. Staff did not keep the children in the classroom with in sight while changing other children's diapers. Staff A had back to the children while changing diapers. Staff B was in the bathroom with a child.
2. Staff A did not clean or sanitize the diapering surface after changing children's diapers.
3. Administrator verified that staff member's back was to the children while diaper was being changed and that the diapering surface was not cleaned and sanitized after each use.
4. These observations were made on review of the classroom footage time stamped 11:53:00-11:57 AM.
