Inspector Notes

A complaint inspection was conducted on 10/27/2022 from 12:25 PM until 3:15 PM, The investigation included staff and children interviews and interaction, observation and review of records. The inspector reviewed compliance in the areas of administration, staffing and supervision, programming, behavior guidance, and special care.

A preponderance of evidence did not support the allegation of unacceptable disciplinary practice or forbidden action.
Information gathered during the inspection did determined non-compliance with applicable standards and a violation was documented on the violation notice issued to the program.

Based upon staff interview, the facility has not ensured that children under ten years of age are always within actual site and sound supervision of staff.\
Evidence:
According to staff interviewed, child 1 receives behavior health services from a therapist while at the licensed facility. Staff confirmed that on occasion the therapist works alone with child 1 without the supervision of facility staff.
Plan of Correction: The facility responded with the following written plans of correction:
Consistently ensure that individuals from independent contractors, when in the presence of children, remain within sight and sound supervision of a staff member in accordance with 22VAC 40-285-230
