Inspector Notes

An unannounced inspection was initiated on 09/09/2024 and completed on 09/10/2024 in response to a complaint received by the licensing office on 09/03/2024 relating to allegations of transportation of children. The inspector reviewed 1 child?s record on site on 9/9/2024 and 2 staff records at the main office on 9/10/2024. This inspection included:

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non- compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/27/2024. A POC submitted after this date will not appear on the public website

Standard 8VAC20-780-180-C
Staff who drive a vehicle transporting children shall disclose any traffic violation that occurred during employment or assignment as a driver.

The most recent documentation of reviewing the driving record with staff 1 who transports children to and from the facility 1 was from nine years ago when staff 1 was hired.
Plan of Correction: The facility responded with the following:
We will continue to review driving records online. We will print driving records every six months for personnel file.
Standard 8VAC20-780-245-H
Annual training shall be relevant to the staff's job responsibilities and the care of children and include topics such as precautions in transporting children.

There was no documentation of training within the past eight years regarding precautions in transporting children in the record provided for Staff 1, who was driving the bus on 9/4/2024.
Plan of Correction: The facility responded with the following:
Itemized training to include documentation relevant to job responsibilities (driver/monitor), Job descriptions will be reviewed.
Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care and protection.

According to staff 1 and staff 2, on 9/4/2024 upon arrival at the bus at the bus stop to drop off children, staff 1 told child 1's parent that child 1 was not on the bus.
After leaving the bus stop, staff 1 and 2 realized that child 1 was on the bus.
Staff 1 contacted the parent and was in communication with the facility administrators.
Plan of Correction: The facility responded with the following:
Bus Monitors will take the lead on tracking which children are on the bus constantly updating the Child Transportation Log when students arrive and exit the bus. Name to face recognition and pictures of students on every bus.

The required first aid kit wherever children are in care shall include a working thermometer and tweezers.

The first aid kit on the full size school bus used to transport children on 9/4/2024 did not include a thermometer and tweezers.
Plan of Correction: The facility responded with the following:
First aid kits were reviewed and missing items were replenished, A first aid checklist will be reviewed monthly.

The center shall ensure that during transportation of children there is a list of names of children being transported in the vehicle.

Staff 1 confirmed that on the afternoon bus run on 9/4/2024, there was not a list of children being transported on the large school bus
Plan of Correction: The facility responded with the following:
We review name to face recognition in addition to placing student's pictures above their assigned seats on all buses. Bus Monitors will update Child Transportation Log. Bus Monitors will go inside to receive students and take initial attendance.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Staff 1 stated that on 9/4/2024, a written record of attendance was not kept as children were boarding the school bus and as the children were departing at their bus stop.
Plan of Correction: The facility responded with the following:
We will ensure all staff is properly using Child Transportation Logs. We have conducted meetings to include both Transportation and Education teams.
