Inspector Notes

An unannounced monitoring inspection was conducted on 11/22/2022 from 2:35 pm - 4:50 pm. At the time of entrance there were 98 children in care with 18 teaching staff. The children in care ranged in age from infant - five years. Children were observed during nap and afternoon snack. Two diaper changes were observed.
Records reviewed for three children and three staff.
The inspector additionally investigated a self - reported incident of alleged inappropriate behavior guidance which could not be substantiated.

The information gathered resulted in violations in the areas of staff background checks, staff TB screenings and building maintenance. These violations are listed on the violation notice issued to the facility.

Based on record review and interview, the center failed to obtain a copy of the results of the results of a criminal history record information check from any state in which the individual has resided in the preceding five years.

Evidence:
1. Staff 1 lacked the results of a criminal record check from another state.
a. Staff 1 has a hire date of 03/07/2022 and has resided outside of Virginia within the past five years according to documentation on file in the employment record.
2. Administrative staff confirmed that an out-of-state criminal record check had not been obtained for staff 1.
Plan of Correction: Background check/out-of-state submitted 1/12/23.
Standard 8VAC20-780-160-A
Based on record review, the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and shall have been completed within the last 30 calendar days of employment.

Evidence:
1. The TB screening for staff 1 was conducted more than 30 days before her hire date of 03/07/2022. The TB screening on file was conducted on 08/31/2021.
Plan of Correction: Update TB & maintain TB protocol moving forward.
Standard 8VAC20-780-270-A
Based on observation, the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a safe condition.

Evidence:
1. Three sections of what appears to be a landscaping hose is exposed in such a way as to create a tripping hazard on the outdoor play area.
a. This area is located near the gate used by children and staff to enter and exit from the outdoor play area.
Plan of Correction: Mulch has been ordered to cover hose.
