Inspector Notes

An unannounced monitoring inspection was initiated on 4/24/2025 and completed on 4/24/2025 in response to a self-report received by the licensing office on 4/16/2025 relating to behavior guidance. The inspector reviewed 1 child's record and 1 staff record on-site on 4/24/2025. This inspection included:

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 05/28/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-400-B
Behavioral guidance shall be intended to redirect children to appropriate behavior.
Staff 1 did not make any attempt to redirect child 1 as their behavior escalated during a tantrum on the afternoon of 4/15/2025.
Plan of Correction: 1. Developed a training series that focuses on how to appropriately respond to challenging behaviors.
2. 3-hour training with a clinical and school psychologist on April 17, 2025.
3. 1- hour training from a VDOE licensing expert on June 18, 2025.
4. Develop behavior specific action cards for teachers with strategies and steps to address specific challenging behaviors ( June 2025).
Standard 8VAC20-780-410-1
The following actions are forbidden: roughly handling a child.
During child 1's tantrum, staff 1 rough handled child 1 by pushing the child away, pulling the child's arm and picking the child up off the floor.
Plan of Correction: 1. Terminated teacher immediately (April 17, 2025) for not following The New E3 School policies and procedures and VDOE licensing standards.
2. Created a three-step procedure for teachers & staff to follow when children are experiencing severe tantrums. Every staff member received one-on-one training + coaching directly related to managing behavior.
