Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 07/28/2025 and completed on 07/28/2025. The on-site inspection began at 9:05 am and ended at 12:10 pm. The inspector reviewed compliance in the areas listed above. There were 100 children present and 19 staff. The inspector reviewed 8 children's records and 5 staff records on-site on 07/28/2025.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 08/08/2025. A POC submitted after this date will not appear on the public website.

Where playground equipment is provided the fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use. The fall zone shall be under equipment with moving parts and meet the use zones required by the National Program for Playground Safety.
The fall zone surrounding the see saw, located on the children's playground, does not meet the requirement of 6 feet on all sides outlined by the National Program for Playground Safety.
Plan of Correction: See saw is being removed. Sod will replace woodchips in that area.
