Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on August 23, 2024, as a part of the licensure period. The on-site inspection began at 8:00am and ended at 9:25am. The inspector reviewed compliance in the areas listed above. There were 5 children, 1 provider and 1 caregiver present; the point total was 15. The inspector reviewed 4 children?s records, 1 provider record, 1 caregiver record and 3 household members record on-site. This inspection included document review, tour of the home, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations

The provider, household member #1, #2, and #3, were missing update fingerprint results on record. The provider used the incorrect form to request the background checks.

The provider confirmed during the interview, the provider and the assistant did not obtain the required sixteen hours of annual training relevant to their job descriptions.

The provider confirmed during the interview, the attendance was not recorded since 8/16/2024.

Child #1 was missing the required information of one designated person to contact, in case of an emergency, if the parent cannot be reached.
Evidence:
Child #1 was playing in the non-mobile bouncy activity toy. The provider did not wash the hands of Child #1, placing them in the highchair, before eating breakfast.

The provider confirmed there was Aquaphor and Butt Paste that was not labeled in the diaper changing area.

The provider confirmed during the interview that, the instructional manual was missing from the first aid kit.
