Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on August 23, 2024, as a part of the licensure period. The on-site inspection began at 8:00am and ended at 9:25am. The inspector reviewed compliance in the areas listed above. There were 5 children, 1 provider and 1 caregiver present; the point total was 15. The inspector reviewed 4 children?s records, 1 provider record, 1 caregiver record and 3 household members record on-site. This inspection included document review, tour of the home, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review, the caregiver did not ensure all caregivers and household members had the required background checks.
Evidence:
The provider, household member #1, #2, and #3, were missing update fingerprint results on record. The provider used the incorrect form to request the background checks.

Based on record review and interview, the provider did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Child #1 was playing in the non-mobile bouncy activity toy. The provider did not wash the hands of Child #1, placing them in the highchair, before eating breakfast.

Based on observation, the provider did not ensure when topical skin products such as diaper ointment are used, if provided by the parent, the product was labeled with the child?s name.
