Inspector Notes

A complaint investigation was begun today in response to an allegation being received in our office. The investigation is being conducted jointly with local authorities.

Based on multiple interviews, the preponderance of evidence supports the allegation of a forbidden action being used on a child. The complaint is valid.
An additional violation was cited that was found during the course of the investigation, but not part of the original complaint.

Based on a review of records, one staff record does not contain documentation of a repeated Central Registry Search (CRS) within the allowable 5 year time frame.
Evidence:
The documentation available for review shows a CRS search dated 11/01/17 (expired 11/1/22). (Staff A)
Plan of Correction: We will send out the documents needed to be in compliance.
Standard 8VAC20-780-410-1
Based on multiple statements, it was determined that a staff member used a forbidden action when dealing with a child.
Evidence:
Staff A used physical punishment (pinching the nose) on a child in an attempt to correct how he was sitting in the chair. The pinch resulted in a mark/bruise on the nose.
Plan of Correction: [Staff member] was fired on January 3, 2023 due to her inappropriate behavior.
