Inspector Notes
An unannounced renewal visit was conducted today. All rooms were in ratio during the inspection
Children were observed playing on the playground, working with the letter "E"', enjoying dramatic play and during circle time.
Emergency evacuation drills, emergency supplies, first aid supplies, menu and posted documents were all in compliance.
4 staff and 7 child records were reviewed. See violation notice for non-compliances cited.
Model/sample forms will be sent for Shelter-in-place and lock-down drill documentation, injury prevention plan, on-going training log.

9Systemic Deficiency
Based on a review of records 2 staff records did not contain documentation of a completed Central Registry Search (CRS) within 30 days of employment.
Evidence:
1. Staff C's record did not contain documentation of a completed CRS. (Date of Hire (DOH): 2/27/23.
2. Staff D's record did not contain documentation of a completed CRS (DOH: 1/16/23)
Evidence:
3 of 4 staff records reviewed contained orientation documentation on an outdated form.
Missing information included prevention and response to emergencies due to food and other allergic reactions. The form does not document the method of delivery for the orientation training. (Staff B,C, D)
Evidence:
The training log for tracking training, does not include the method of delivery for the training.
Based on a review of medications, the center failed to follow its own policy regarding informing parents of an expired medication.
