Inspector Notes
A monitoring inspection was initiated on 8/8/2023 and concluded on 8/8/2023. There were a total of 9 children ( 28 points) observed with 3 caregivers present. The children were engaged in the following: toy play, and infant napping. The child care area, 5 child records, 4 caregiver/household member records, and evacuation drills were inspected. The Inspector recommended that the inside climber that does not meet standards and staff say the children do not use, be removed from the childcare area to eliminate possible harm to the children in care. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. 4Evidence: Caregiver #3 TB screening was dated 7/29/2021 and has not been updated 2 years from the date of 7/29/2021.
Evidence: Two of the adult pink chairs against the wall has a screw that is loose which could possibly cause a pinching hazard to the children in care.
Based on a review of children records, some children records did not contain all the required
information.
Evidence: Child #1, date of first attendance 7/1/2022 is missing parent authorization for
medical care.
Child #2, first day of attendance 7/5/2022 is missing physical examination.
Based on a review of children records, the family day home has not obtained
documentation of additional immunizations once every 6 months for children under the
age of two years.
Evidence: Child #3, date of birth 5/7/2022 does not have documentation of updated
immunizations every 6 months. Child's last documented immunization was 5/2022.
