Inspector Notes
An unannounced monitoring inspection was conducted on February 9, 2024, from 10:05 am-11:15am. At the time of entrance, ( 9 ) children (29 ) points were in care with (3 ) caregivers present. The inspector reviewed 3 children?s records and 5 caregiver/household member records. Children were observed during snack time, and outdoor play time. No medications were on site. Areas of non-compliance are identified on the violation notice. Repeat violations were found and are identified within this report.

Household #2 does not have a updated TB screening in the record. The original TB screening was not available to be reviewed.
Based on child record review, 1 child's file did not have 2 emergency name, address and phone number for an emergency contact.
Evidence: Child # 1, date of enrollment 10/4/2023 does not have a second name, address and phone number to contact in case of an emergency.
Evidence: Child #2, date of enrollment 1/2/2024 does not have all the required written acknowledgement forms from the parent.
