Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

A monitoring inspection was initiated on July 26, 2021 and concluded on July 28, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 2 children present, ranging in ages from birth to 12 years, with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 4 household members records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of four household member records, one does not contain the required documentation.
Evidence:
The record for household member number one does not contain a current report of tuberculosis screening form. The current form expired in April 2021.
Plan of Correction: The tuberculosis screening form will be completed and placed in household members record. I will ensure all screenings are completed before they expire.
