Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

A monitoring inspection was initiated on January 10, 2022 and concluded on January 12, 2022. The provider was contacted by telephone and a virtual inspection was conducted. There were 3 children present, ranging in ages from newborn to 12 years with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of household member records, the provider did not obtain for each household member a current report of tuberculosis screening form every two years from the date of the first screening.
Evidence
Household member #1 has a report of tuberculosis screening form that expired 4/21.
Plan of Correction: I will get a copy of his updated tuberculosis screening form.
