Inspector Notes
An unannounced, on-site renewal inspection was initiated on 7/2/2025 and completed on 7/2/2025, as a part of the licensure period. The on-site inspection began at 9:38AM and ended at 11:39AM. The inspector reviewed compliance in the areas listed above. There were 54 children present and 8 staff. The inspector reviewed 4 children?s records on site on 7/2/2025 and 4 staff records on 7/2/2025.
This inspection included - document review, tour of the facility, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-780-260-A
The center shall provide an annual fire inspection report from the appropriate fire official. The annual fire inspection report was overdue by 1 month.
Plan of Correction: Provider will schedule inspection to be completed.
Standard 8VAC20-780-280-B
REPEAT VIOLATION
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. A bottle of air freshener with a warning label, ?Keep out of reach of children,? was unlocked in the school age classroom, within reach of children.
Plan of Correction: Provider locked air freshener in cabinet.
Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care and protection. Child #4, age 5 months, was placed in a bouncer with their bottle propped up using a small folded fleece blanket. Staff #4 placed the blanket approximately 3 inches from the child?s face.
Plan of Correction: Provider will ensure staff hold infants that cannot feed themselves.
Standard 8VAC20-780-570-B
Bottle fed infants who cannot hold their own bottles shall be held when fed. Child #4, age 5 months, was placed in a bouncer by Staff #4 with their bottle propped up using a small folded fleece blanket. The child could not hold the bottle on their own.
Plan of Correction: Provider will ensure staff do not prop bottles for infants.

The following staff records shall be kept for each staff person: the following required components were missing. Staff #1 record, employed 1 month, did not have documentation of a job title.
