Inspector Notes

An unannounced monitoring inspection was conducted on October 4, 2023, from 9:20am-10:45am. At the time of entrance, ( 22) children were in care with (5 ) staff members present. The inspector reviewed 3 children?s records and 3 staff records. Children were observed finishing up snack, and bathroom breaks. One medication was observed. Areas of non-compliance are identified on the violation notice.

Standard 8VAC20-780-160-A
Based on a review of staff records, staff did not submit a negative TB screening within the last 30 calendar days of the date of employment.
Evidence: Staff # 2 was hired 2/27/2023, the TB screening was completed on 4/20/2023, which was 2 months after she had begun employment.
Plan of Correction: Staff will submit a TB screening prior to their employment.
Standard 8VAC20-780-240-C
Based on staff record review, staff have not completed the required training on recognizing child abuse and neglect prior to the staff working alone with children.
Evidence: Staff # 1 and staff #2 were observed alone in the classroom today caring for children. There was no documentation in the staff's records that they have taken the child abuse and neglect training.
Plan of Correction: Staff will take the training and provide a certificate for their staff record.
Standard 8VAC20-780-270-A
Areas and equipment is not maintained in a safe and operable condition.
Evidence: The step on the rock wall slide, located on the large playground is broken, exposing sharp edges to the children in care.
Plan of Correction: Children will not be allowed on the rock wall climber until the step can be removed from the climber.
Standard 8VAC20-780-340-D
Based on observation and staff record review, there are not qualified program leaders in each grouping of children.
Evidence: The staff observed caring for children alone in the crawlers (staff # 3), young toddlers (staff # 2) and Preschool - Pre K ( staff # 1) classrooms are not qualified program leaders based on their documentations found in the records.
Plan of Correction: Staff are working on getting their CDA's.
