Inspector Notes
An unannounced monitoring inspection was conducted on April 9, 2024, from 9:20am-10:50 am. At the time of entrance, ( 24) children were in care with ( 7 ) staff members present. The inspector reviewed 3 children?s records and 6 staff records. Children were observed during snack time, engaged in group circle time and outdoor play time. No medications were onsite. Areas of non-compliance are identified on the violation notice. Repeat violations were found and are identified within this report.

Violations
3Based on observation, areas and equipment is not maintained in a safe and operable condition.
Evidence: The step on the rock wall slide is broken, exposing shard edges to the children in care. The rock wall slide is located on the large playground, the slide closest to the center and the wall facing the wooden fence.
Based on observation and staff record review, there are not qualified program leaders in each grouping of children.
Evidence: The following staff were observed caring for children alone in the Young Toddlers (staff # 3), Crawlers (staff# 5) and Preschool/Pre K ( staff # 1) are not qualified program leaders based on their documentation found in the records.
Evidence: There was no documentation of a fire evacuation drill for March 2024.
